Use Disorder Assessment

A good start in determining whether you or a loved one has a substance use disorder (SUD) can be found in the following questionnaire and screening tool. These are by no means a replacement for professional assessment, but they can help start the process toward treatment and recovery.

  • Questionnaire courtesy of the National Institute on Drug Abuse
  • Screening for alcohol use disorders developed by the World Health Organization
NOTE: Alcohol use disorder often co-exists with opioid use disorder
